I guess you covered almost all common reasons for getting your site de-indexed. I have seen many people making a basic mistake in robots.txt
User-agent: *
Disallow: /
Would mean you have disallowed the bots to index your website. Luckily most of the times google webmaster tools can tell you exact reason for de-indexing.
Sometimes sites are de-indexed when they get penalized by big G for not following the guidelines. I won’t say that buying / selling link is ‘black-hat’ so you might want to add this as a separate point to your post

Your tips are correct. By my experience, I mainly get de-indexed because the time of loading pages is too long (flash, big and not optimized photos etc.)

I read in a forum recently that a search engine spider is like a Navy Seal. In and out so I agree with what you said, if it takes to long the spider is gone

I find the best way to climb out of the google sand box is to write articles, submit to google’s blog carnivals, and to comment on other peoples blogs. All three will lead to increased links which will boost page rank and will get you indexed. If yoe do not have enough time to keep fresh content rolling in then hire an inexpensive ghost writer to do it for you!

Great Check list. Who can stay calm though. Does a dedicated IP give better ranking. I’ve seen it both ways.
